Monthly Payment Plan

Lawrence offers the option of interest-free monthly payment plans, allowing families to spread payments over 10 months. 

How to Enroll

  1. Complete the Monthly Payment Plan Enrollment Form (available after July 15th)
  2. Submit the enrollment form along with your first payment to the Student Accounts Office

FAFSA Verification

Form Description
Dependent Student Verification Form Complete this form if your FAFSA was selected by the US Dept. of Education for "verification" and you were instructed by the Financial Aid Office to complete this form.

 

Student Employment Forms

The following forms should be submitted to the Payroll Office located in 115 Brokaw Hall.

Form Description
I-9 Form (PDF)

All employers must complete Form I-9 to document verification for the identity and employment authorization of each new employee. 

  • Complete pages 7 & 8
  • You must submit this form IN-PERSON
  • You must have original documents (see page 9) when submitting this form.
W-4 Form (PDF)   Complete this form so that your employer can withhold the correct federal income tax from your pay.
Direct Deposit (PDF)

Students eligible for Federal Work-Study are strongly encouraged to enroll in Direct Deposit. All other students are required to enroll in Direct Deposit.

Payroll Deduction Authorization (PDF) Earnings are paid directly to the student. Complete this form if you would like some or all of your student employment earnings to be paid to your student account.
