Report a Bias Incident

If you witness or are the victim of a bias incident, please report it here. If you submit your name, you will be contacted to discuss the incident further before any action will be taken.

The purpose of this form is to track instances of bias incidents on and off campus. Examples may include but are not limited to the use of degrading language or slurs, spoken or written. Incidents that rise to the level of a crime should also be reported to Campus Safety or Appleton Police. This form does not take the place of reporting a crime to law enforcement.

Anonymous reporting is permitted but limits the ability for someone to follow-up on the concern.
A report submitted anonymously will be reviewed by the Provost.
