New Student Forms

Form	Description
Federal Student Aid Authorization Form	This form is also available in Voyager. We recommend completing it electronically. Should you have a "credit balance" (a negative balance), you have more financial aid then the billed charges on your account. If you receive Federal Student Aid, you must instruct Lawrence what to do with your credit balance (leave it on your account, pay past due charges, or refund the balance to you).
Student Payment Agreement & Disclosure Statement	All students must complete this form and return it to the Financial Services Office (which sounds a lot like the Financial Aid Office, but is actually just down the hall).

Form	Description
I-9 Form	All employers must complete Form I-9 to document verification for the identity and employment authorization of each new employee. Complete pages 7 & 8. You must have original documents (see page 9) when submitting this form.
W-4 Form	Complete this form so that your employer can withhold the correct federal income tax from your pay.
Direct Deposit	Students eligible for Federal Work-Study are strongly encouraged to enroll in Direct Deposit. All other students are required to enroll in Direct Deposit.
Payroll Deduction Authorization	Earnings are paid directly to the student. Complete this form if you would like some or all of your student employment earnings to be paid to your student account.
