Student-Athlete Info

Any student who plans on participating with any intercollegiate athletic team at Lawrence University must have the required medical forms and insurance information completed and on file in the Athletic Training Room before being allowed to participate in any practice, contest, or tryout.

All forms are below and/or on the Athlete Portal once the student-athlete logs in.  Please see the checklist for all forms needed and their locations.  If you are unable to download or print these forms from the Athlete Portal, please contact the Head Athletic Trainer at 920-832-6762 or jami.l.rogers@lawrence.edu.

Incoming Freshmen and Transfer Student-Athletes

All forms must be completed prior to your first participation.  There are no exceptions.  Fall athletes, all of your forms and online updates need to be submitted by August 1 in order for you to be fully cleared to participate on the first day or practice.  Winter and Spring Athletes, your forms are due September 1.  

Please make sure you review the insurance requirements to make sure they have been met.

All new student-athletes are required to have a physical within six months of either (1) the first day of practice for fall sports or (2) September 1 for winter and spring sports.  For mid-year transfer students, your physcial date must be within six months of your first day at Lawrence.  It is the responsibility of the student-athlete to deliver a duplicate copy of the physical to the Athletic Department.

Returning Lawrence University Student-Athletes

Fall Sport Returners:

If you received a free physical screening on campus in May and were cleared by the doctors, you do not need to get another physical from your doctor at home this summer.  If you were told by the doctors that you have something you need to follow-up on, please do so and return updated paperwork after the follow-up.  You must update any information that may have changed on ATS.

Winter and Spring Sport Returners:

Dates and times for free physical screenings will be sent out to you from your coaches in the beginning of September.  They will most likely take place during the first week of school.  If you are unable to make the physical screening, you are fully responsible for obtaining a physical on your own and are responsible for the costs incurred to you or your insurance.  You must update any information that may have changed on ATS.

Returning Your Forms

All forms can be filled out on or uploaded via the online Athlete Portal.  If you have problems with uploading or wish to provide in an alternate way, they can be delivered to the Head Athletic Trainer in one of three ways:

You only need to send them in one manner, for example, if you fax a form over, you do not need to mail the hard copy.  You will be informed if a form is unreadable or if we need an additional copy.


Important note:

There is only one form used by Lawrence University Health Services and Lawrence University Athletic Training.  This is the “New Student Medical History and Physical Form”.  All other forms are completely separate and should be treated separately from any forms required by the Lawrence University Health Center.
