This form is only for requests to stay on campus between November 27, 2013 - January 5, 2014. Only requests from students who live outside the Fox Cities region and who maintain employment where that job provides at least 20 hours of work per week will be considered.
Winter athletes and fall student teachers should not fill out this form.
International Students do not need to complete this form. All international students should contact Leah McSorley, Director of International Student Services, at leah.d.mcsorley@lawrence.edu regarding winter break arrangements no later than November 1, 2013.
