Session Logs
There are 3 types of logs to submit, depending on the type of tutoring request.
Please complete the appropriate form from the list below.

- Single student tutoring, request already exists (was sent to you by the CTL) -
Choose this form if the CTL sent you the original tutoring request for ONE student. - Single student tutoring, new request (did not originate from the CTL) -
Choose this form if the CTL DID NOT send you the original request for ONE student. - Group tutoring session -
Choose this form if you held a group tutoring session.
