Required forms for new students who are applying for financial aid.

Verification
If your FAFSA is selected for Verification, the U.S. Department of Education requires your school (Lawrence) to verify certain information reported on your FAFSA.
Special Circumstance Form
Enrolling Students
Forms for students who have officially decided to enroll at Lawrence.
Required Forms
1. Free Application for Federal Student Aid (FAFSA)
All students applying for need-based aid must complete this form annually. The federal school code for Lawrence University is 003856.
2. Supplemental Financial Aid Application
All new students applying for financial aid must submit the Lawrence University Application for Financial Aid OR the CSS/Financial Aid Profile.
3. 2011 Federal Tax Return
Submit a copy of Pages 1 & 2 of IRS Form 1040, 1040A, or 1040EZ and, if applicable, Schedules A, C and/or E.
4. 2011 W-2 Forms
Submit copies of all 2011 W-2 Forms
5. If applicable: Non-Custodial Parent Financial Aid Form
To be completed by the non-custodial parent in the case of a dependent student whose parents are divorced, separated, or were never married. (The custodial parent should complete the FAFSA and the supplemental financial aid application.)
Conditional Forms
Independent Student Financial Aid Application
If you are married, 24 years old or older, have children or dependents you support, or meet other requirements, you are considered "Independent" when completing the FAFSA.
Non-Custodial Parent Waiver Form
The “non-custodial parent” is the parent whose information is not reported on the FAFSA. At Lawrence, financial information is required from both birth parents to determine a student’s eligibility for need-based financial aid. By completing this form, you are requesting that we waive the requirement to obtain information from your non-custodial parent.
Dislocated Worker Verification
If you have reported on your FAFSA that you, your spouse, or your parent is a Dislocated Worker or a Displaced Homemaker, you must submit this form.
If you indicated on the FAFSA that you are an Unaccompanied Homeless Youth or a youth that is self-supporting and at risk of being homeless, you must complete this form. Lawrence policy requires that a copy of the official determination of a student’s status under the McKinney-Vento Act be submitted prior to processing a financial aid award.
Forms for Enrolling Students
Stafford Loan Entrance Counseling
If you are a first-time Stafford Loan borrower, you must complete Entrance Counseling.
Stafford Loan Master Promissory Note
If you are a first-time Stafford Loan borrower, you must complete a Master Promissory Note.
Perkins Loan Entrance Counseling
If you are a first-time Perkins Loan borrower, you must complete Entrance Counseling.
Perkins Loan Master Promissory Note
If you are a first-time Perkins Loan borrower, you must complete a Master Promissory Note.
All students who receive any Title IV funds (Pell, SEOG, Perkins, or Federal Direct Student Loans) as part of their financial aid package are required to complete a Title IV Authorization form. Your authorization form provides the University instructions on how to apply excess Title IV funds to your student account. Excess Title IV funds are created when the total amount of Title IV funds posted to your student account exceeds charges for tuition, student fees, room and board.
Student Payment and Disclosure Statement
The Federal Truth-in-Lending Act requires complete disclosure of the terms and conditions controlling payment of the student's obligations. In order to comply with those federal statutes and regulations, the University requests that the student carefully review this document before signing it. The completed form can be mailed, faxed, emailed or turned in at the cashier window in Brokaw Hall.
Complete this form if you are receiving scholarships and/or grants from sources other than Lawrence University
