Student Payment Agreement & Disclosure Statement
The Federal Truth-in-Lending Act requires complete disclosure of the terms and conditions controlling payment of the student's obligations. In order to comply with those federal statutes and regulations, the University requests that the student carefully review this document before signing it. The completed form can be mailed, faxed, emailed or turned in at the cashier window in Brokaw Hall.
Student Consent to Release Information
The Family Education Rights and Privacy Act (FERPA) of 1974 is a federal law that protects the right to privacy and confidentiality of student records. Schools must have written permission to release student record information. If you wish to authorize the Lawrence University Office of Financial Aid or Student Accounts Office to disclose information to a third party (e.g., parents, siblings, spouse, etc.), please complete and sign this form. We will disclose some financial aid and student account information without receipt of this form to a parent whose information is reported on the FAFSA or who has claimed the student as a dependent on their most recent federal tax return.
