Forms

This page contains forms used by the Education Department at Lawrence University to administer our teacher certification programs. (Equivalent forms specific to music education can be found on the Conservatory of Music website.)

Pre-Student Teaching Practicum

100 hours of practicum are required prior to student teaching. The following forms are distributed in teacher education courses, each of which requires a 20-hour practicum.

• Principal Letter - given by the student to the school secretary when arranging a practicum
• Teacher Letter and Practicum Signature Form - given to the mentor teacher when arranging a practicum; the signature form is to be signed by the mentor teacher and returned to the course professor
• Practicum Self-Assessment - completed by the student at the end of the practicum
• Practicum Evaluation Form - completed by the mentor teacher at the end of the practicum; this form certifies the practicum hours
• Multicultural Education Practicum Evaluation Form (for EDST 350) - completed by the mentor teacher at the end of the EDST 350 practicum (in place of the form above)

Admission to the Teacher Education Program

Students who wish to apply to the Teacher Education Program should pick up a Teacher Certification Handbook from the department office (M-F mornings before 11:30).

• Petition for Student Teaching - completed by the student and submitted to the education office after completing at least one education course and passing Praxis I
• Recommendation for Admission to the Student Teaching Program - completed by recommending faculty; three recommendations are required, including one from the advisor
• Student Teaching Interview Evaluation Form - completed by the Teacher Education Committee after interviewing the candidate

Student Teaching

The following forms are completed when a student teacher is observed or evaluated during the student teaching semester.

• Student Teacher Observation Form - completed by the university supervisor when observing a student teacher; copies are given to the student teacher and cooperating teacher
• Student Teaching Evaluation Form - completed (separately) by the cooperating teacher and university supervisor for midterm and final evaluations
