The statement of intent form must be submitted to Faith Barrett by 3 p.m. on Friday April 4, 2008 (or by the end of the first week of classes during the term in which you intend to file your project). Please leave your form in Professor Barrett's mailbox in the Main Hall faculty office (first floor).
You may print this form using your browser or, alternatively, you may obtain a printed copy of the form from the Reference Desk in the Library. However you obtain the form, fill it out by supplying the requested information, sign it yourself, obtain your advisor's signature, and then deliver it to Ms. Barrett by the deadline.
