This page lists forms used by the Education Department to administer our teacher certification programs. (Equivalent forms for music education can be found on the Conservatory of Music website.)

Admission to the Teacher Education Program

Students who wish to apply to the Teacher Education Program should pick up a Teacher Certification Handbook from the Education Department office (Briggs 117, M-F 7:30-11:30 a.m.) along with the first two forms listed below.

Application / Petition for Student Teaching (PDF) - This program application must be completed by the student and submitted to the Education Department.  Before applying, the student must complete one education course and attain a qualifying score on a standardized academic skills test (Praxis I PPST, Praxis Core, ACT, SAT, or GRE).

Recommendation for Admission (PDF) - This faculty recommendation form must be sent to the Education Department to complete the student's application.  Three recommendations are required, including one from the student's advisor.

Admission Checklist (PDF) - This form is completed by the Teacher Education Committee after the application review and candidate interview.  A copy is sent to the candidate along with the admission letter.  Candidates who are denied admission may be invited to reapply when requirements are met (increased GPA, qualifying test scores, etc.).

Course Practicums

A minimum of 100 practicum hours are required prior to student teaching. The following forms are distributed in teacher education courses, each of which requires a 20-hour practicum.

Principal Letter (PDF) - This letter is given by the student to the school secretary when arranging a practicum; it identifies the student as enrolled in a Lawrence teacher education course.

Teacher Letter & Practicum Signature Form (PDF) - This letter is given by the student to the mentor teacher when arranging a practicum; the signature form must be returned to the course professor by the end of week 2 of the term.  For multiple placements, a separate form is required from each teacher.

Practicum Self-Assessment (PDF) - The form is used to log practicum visits, identify activities, and reflect on the practicum experience.  It is completed by the student and submitted to the course professor by week 10 of the term.  Practicum self-assessments are kept on file in the Education Department and included in the candidate's pre-student teaching portfolio.

Practicum Evaluation (PDF) - This form is used to certify the number of hours completed and rate the student's performance.  It is completed by the mentor teacher and submitted to the course professor by week 10 of the term.  Practicum evaluations are kept on file in the Education Department and included in the candidate's pre-student teaching portfolio.

Multicultural Education Practicum Evaluation (PDF) (for EDST 350) - This form is used in place of the form above.  It includes additional ratings related to ethnicity, cultural diversity, and education.

Pre-Student Teaching Review

The following form must be completed before a student may begin student teaching.

Pre-Student Teaching Checklist (PDF) - This checklist is completed by an education faculty member when reviewing the candidate's pre-student teaching portfolio.  The completed checklist is kept in the student's file in the Education Department.

Student Teaching

The following forms are completed when a student teacher is observed or evaluated during the student teaching semester.

Student Teacher Observation (PDF) - This form is completed by the university supervisor when observing a student teacher.  Copies of the completed form are given to the student teacher and cooperating teacher, and the original is placed in the student's file in the Education Department.

Student Teaching Evaluation (PDF) - This form is completed separately by the cooperating teacher and the university supervisor for the midterm (9-week) and final (18-week) student teaching evaluations.  Completed forms are reviewed and signed by the student teacher and placed in the student's file in the Education Department.  Final evaluations are included in the candidate's final teaching portfolio.

Certification Review

The following form is completed when the candidate has finished student teaching and completed all other program requirements.

Certification Checklist (PDF) - This form is completed by the Director of Teacher Education to certify the candidate for initial licensure.  The completed checklist is kept in the student's file in the Education Department.
